Spaghetti can be replaced with fettuccine or other long pasta. Cream can be taken with a lower fat content - from 20%. You can replace Parmesan with other dry or semi-dry varieties of cheese. If the maximum similarity with Italian pasta is not necessary for you, you can replace Parmesan with ordinary Russian-type cheese.
